For becoming member of HSI / HSI + APHS send

a) Completed form
b) Self-attested photocopies of educational qualifications,
c) A letter from the HOD if applying for Associate membership
d) A passport sized colour photograph and
e) Demand draft for the appropriate amount [Rs 11,800 (HSI + APHS, including 18% GST for HSI membership) or Rs 3,540 (HSI only, including 18% GST)] in favour of “Hernia Society of India” payable at New Delhi to Dr Manish Baijal, Hon. Treasurer, HSI Institute of Minimal Access, Metabolic and Bariatric Surgery (East Wing 5th Floor), Max Super Speciality Hospital, 2, Press Enclave Road, Saket, Delhi-110017
